This information is written in a way that is easy to read. If you would like help to read or understand this document, a family member, friend, support person, advocate or worker may be able help you.
What is this document for?
Oakbridge Coordination & Support needs some information about you so we can support you. This may include personal information such as: Your name, your date of birth, your address, contact information and information about your disability, health, and supports. You may prefer that other people do not know this information.

Your personal information
We only collect information that we need to support you. If we need information from someone else (for example a doctor, therapist, school, or another provider), we will ask for your permission first. You can ask us what information we have about you at any time.
How we keep your information safe
We store your information securely. Electronic files are protected by passwords and security programs. Paper files are kept in a safe place. We will not talk about you with other people unless: you ask us to, or you give us permission.
We will not share your information in public places (such as notice boards) or online.
Photos: we will only take or share photos of you if you agree in writing (for example, for our website or social media). We keep your information only for as long as we need it. After that, we securely delete or destroy it.
When do we have to tell others about you?
Sometimes the law requires us to share information to keep you or someone else safe.
We may need to report concerns to the right services, such as the Police or the NDIS Quality and Safeguards Commission, if: you are at risk of harm, or someone else is at risk of harm, or there is a serious incident that needs reporting.
If we need to share information for safety reasons, we will tell you (and your nominee or support person, if appropriate), unless doing so would increase risk.
