Oakbridge Coordination and Support
Oakbridge Coordination & Support is an NDIS provider delivering Disability Support Services and Support Coordination across Brisbane, Ipswich, Murweh Shire and surrounding communities.
Our team supports participants to navigate complex support environments, overcome barriers, coordinate services and access reliable, person-centred supports that promote independence and strengthen connections with community and mainstream services.
We are committed to strengths-based, trauma-informed and culturally responsive practice. Our approach is flexible, collaborative and goal-oriented, with a focus on building trusted relationships, respecting individual choice and control, and delivering consistent, high-quality supports tailored to each participant's needs and goals.
Our Mission
Our mission at Oakbridge Coordination & Support is to reduce barriers, help participants navigate complex systems and empower people with disability to access quality, reliable and person-centred supports that promote independence, choice and meaningful community participation.
We are particularly committed to improving access to quality disability services in rural and remote communities, where limited service availability and geographical barriers can make accessing the right supports challenging.
We work alongside each person and their support network to understand their goals, provide clear and practical coordination and support, build meaningful connections and ensure services are aligned with their individual needs.
Our approach is strengths-based, trauma-informed and culturally responsive, guided by what matters most to each person and their right to choice, control and inclusion.
