Oakbridge Coordination & Support is a NDIS provider delivering Support Coordination and Psychosocial Recovery Coaching across Brisbane and Greater Ipswich. Our team supports participants to navigate complex support environments by reducing barriers, coordinating services, and strengthening connections with community and mainstream supports.

Our Mission
Oakbridge's mission is to reduce barriers and help participants navigate complex systems with clear, practical coordination and support. We work alongside each person and their support network to clarify goals, connect with the right providers and community supports, and keep services organised and aligned. We work from a strengths-based, trauma-informed and culturally responsive approach, guided by what matters to you.
